Trump Administration Begins Sending $500 ACA Refund Checks to Nearly 1 Million Americans

WASHINGTON — The Trump administration has begun sending $500 Affordable Care Act refund checks to nearly one million Americans, marking a new federal effort to return surplus marketplace fees.

 

The payments target eligible ACA enrollees in 30 states that use the federally operated marketplace, HealthCare.gov, rather than residents of states running their own insurance exchanges.

The White House announced the refunds in September, saying nearly one million people would receive $500 each after federal marketplace operations collected more user fees than necessary.

Health policy researchers note that marketplace user-fee surpluses accumulated over several years, including periods before and during the Biden administration, complicating claims about exactly when the money was collected.

The White House says most recipients do not receive premium assistance and therefore paid the full cost of their ACA marketplace coverage without federal premium tax credits.

The checks are arriving as health insurance affordability has become a significant concern, following the expiration of enhanced ACA premium tax credits at the end of 2025.

KFF estimates average monthly premium payments for ACA marketplace enrollees increased 58 percent in 2026, from $113 to $178, while average deductibles also rose substantially.

The same KFF analysis estimated average marketplace enrollment could fall to roughly 17.5 million people in 2026, compared with 22.3 million during the previous year.

Those changes mean the $500 payments are arriving in a healthcare market where many households are already confronting higher premiums, larger deductibles, or difficult decisions about coverage.

The White House argues that returning the surplus directly to eligible consumers demonstrates its effort to reduce healthcare costs and increase accountability within the federal marketplace.

According to KFF, average deductibles increased by about $1,027 per person in 2026, reaching approximately $3,786 as enrollment shifted toward plans with higher out-of-pocket costs.

The refunds are arriving shortly before the November 3 midterm elections, when healthcare affordability and household expenses are expected to remain prominent issues for voters.

However, the $500 refund does not necessarily correspond to the exact amount any individual enrollee paid in user fees, and eligibility varies according to marketplace participation.
